VR World – Participation Waiver Form

By signing this waiver, you acknowledge and agree to the following terms and conditions for participation in VR experiences at VR World:

• Health Notice: Games may cause motion sickness, dizziness, eye strain, or disorientation. Do not participate if you are pregnant, have a heart condition, are prone to seizures, or have any other medical condition that could be affected by VR. Anyone under the influence of drugs or alcohol should not participate. Consult a physician before playing if unsure.
• Assumption of Risk: You voluntarily accept all risks associated with VR use, including the risk of injury. VR World is not responsible for accidents, personal injury, or equipment misuse.
• Responsibility for Equipment: You are responsible for handling VR equipment with care. Damage caused by negligence may result in a charge for repair or replacement.
• Parental Consent: Games are not suitable for children under the age of 6. Participants under 14 must be accompanied by a parent or legal guardian who accepts responsibility and signs this waiver on their behalf.
• Media Use: We may occasionally take photos or videos for marketing purposes. If you do not wish to be photographed, please inform a staff member before your session.
• Final Note: By entering our play zone, you confirm that you have read and understood this disclaimer and agree to participate under your own responsibility. Supervision of children is the sole responsibility of their parents or guardians. Compliance with the rules and regulations of Memorial City Mall is required.
